SOFT BOUND REPORT SUBMISSION APPROVAL FORM

Download this form and fill in all the necessary details. Student is requested to obtain the signature for his/her respective supervisors and submit together with 2 copies of your soft bound fyp 2 report.

Failure to obtain the signature of the supervisor will cause the student's name to be dropped from the presentation schedule and the student will need to fill in the request for extension form.

 

 

 

 

HARD BOUND REPORT SUBMISSION APPROVAL FORM

Submit your hard bound report together with this approval form. Signature of your supervisor is required. The coordinator will have the right to reject the submission of the FYP 2 report without the supervisor's signature.

 

 

 

 

 

 

REQUEST FOR EXTENSION

Students who like to extend their project have to fill in this form and obtain the approval from their respective supervisor. The allowable duration is 2 months starting from the beginning of the semester and the highest grade that will be given is B.
